The request for access to an administrative or clinico‑administrative document of the CIUSSS du Saguenay–Lac‑Saint‑Jean must be addressed to the Responsible Person for Access to Administrative Documents and the Protection of Personal Information (hereinafter the “Responsible Person”).
An administrative document is a non‑clinical document held by a public body in the exercise of its functions, such as minutes, studies, organizational charts, policies, or contracts. An administrative document may also contain personal information obtained as part of an administrative process (e.g., a pay stub). Access to this personal information is granted only to the concerned individual, except in cases provided by law. When referring to a clinico‑administrative document, this designates any written clinical process or procedure intended for health professionals. These documents do not contain confidential information; they are working documents intended for health professionals.
The request may be verbal or written; however, only a written request allows you to appeal the Responsible Person’s decision to the Commission d’accès à l’information du Québec.

Once a valid request is received, the Responsible Person has 20 calendar days to respond. Upon receipt of your request, an acknowledgment of receipt is issued containing the following information: the date the request was received, the response deadline, and the possible recourses if the delay is not respected or in case of dissatisfaction. The 20‑day delay may be extended by 10 additional days, when necessary, provided the requester is notified beforehand.
